Matrix GEMINI STEM Outreach Consent and Liability Waiver

I give permission for my child to participate in all activities organized by the camp. I understand that all reasonable safety precautions will be taken, and I will not hold the organization, its staff, or volunteers liable for any accidental injuries or illness that may occur.

In the event of emergency, I authorize camp staff to secure proper medical treatment for my child.

Photo and Media Release

I give permission for photos and/or videos of my child to be taken during camp activities and used for promotional purposes, including website, social media, and brochures.
